Health professionals can access our Referrals

District Nursing

Our District Nursing Service provides a range of clinical nursing needs for people in their own home, supporting independence and improving health and wellbeing. The service recognises that every individual has different needs and is respectful of cultural diversity.

Care is delivered by a team of experienced Registered Nurses and Enrolled Nurses. 

The District Nursing Service can provide:

  • continuity of acute nursing care through Post-Acute Care Programs
  • wound care management
  • continence care
  • medication management
  • support with stoma care
  • assistance with diabetes management
  • drain tube management. 

A fee for service is applicable for most services. Fees are determined by the current patient and client fees schedule. 

Referral criteria

  • A referral from a doctor or health professional is required 
  • A client must reside within Bass Coast Shire
  • Relevant clinical information must be provided (eg. recent scans and test results, specialist letters, wound care plans)
  • Patients returning home from public hospital are eligible for post-acute care
  • Please complete the Ambulatory Care Referral August 2024 form (at the top of this page) and email to district.nursing@basscoasthealth.org.au
