Support at Home (Flexihealth)

To be considered for a Support at Home program, consumers must be assessed as requiring one by the Aged Care Assessment Service.
To request an Aged Care Assessment, please self-refer or discuss your care needs with your doctor, your current care provider, family or friends who can refer you for an assessment via My Aged Care.

When you are assessed, you will be advised that you have been added to the National Wait List for a Support at Home program. When you are allocated support, you will be advised of the process, and provided with a letter and a referral code that your nominated provider will require so they can view your information.
Once My Aged Care notify you that you have been allocated support, you have six weeks to locate a Support at Home provider that you wish to manage the services for you. Bass Coast Health's Support at Home Program, known as Flexihealth, is one local provider able to deliver this service for you.

What can Support at Home provide?
Support at Home has eight different levels of care. Which one you are assessed for depends on your care needs and situation.
Each package level has a funding allocation from Medicare to allow your case manager to provide tailored support services.
These services can assist you to remain at home, with supports customised to your individual needs. Services we can provide from your funding allocation include:
Personal and Daily Living Support
• Assistance with showering, dressing, grooming and toileting
• Help with mobility and transferring (e.g., from bed to chair)
• Support with eating and drinking
• Medication reminders and management
• Continence management.
Home and Domestic Assistance
• House cleaning and laundry
• Meal preparation and nutrition support
• Shopping assistance or delivery
• Basic home maintenance and repairs (safety-related)
• Lawn mowing and gardening.
Health and Nursing Services
• District nursing (wound care, medication management, monitoring health conditions)
• Allied health services, such as:
o Physiotherapy
o Occupational therapy
o Podiatry
o Speech pathology
o Dietetics
o Exercise physiology.
Social Support and Community Access
• Companionship and social visits
• Support to attend social activities or community groups
• Assistance to participate in hobbies, volunteering or cultural activities.
Transport and Mobility
• Transport to and from medical appointments
• Help attending social or shopping outings
• Assistance with mobility aids and equipment.
Respite Care
• Short-term care to give carers a break (in-home or community-based).
Care Coordination and Care Management
• Development of your personalised care plan
• Ongoing review and adjustment of supports as your needs change
• Coordination of multiple service providers.
Assistive Technology and Equipment
• Equipment to improve independence and safety, such as:
o Shower chairs, grab rails, walkers, or mobility scooters
o Personal alarms and monitoring devices.

Availability
Flexihealth Support at Home is run from the Bass Coast Health Wonthaggi site and provide support and care management to clients who have nominated Bass Coast Health as their provider.
