Health professionals can access our Referrals
The Chronic Cardiology Clinic is a fortnightly specialist run clinic that aims to support General Practitioners (GP) to manage consumers in the community.
Referrals may come from Bayside Health services, general practice clinics or private cardiologists for people experiencing symptoms associated with, but not limited to, chronic heart failure, ischaemic heart disease and atrial fibrillation.
The goal of this clinic is to optimise management of chronic cardiovascular disease, improve quality of life and reduce hospital admissions.
